Very often we have encountered departments that are experiencing sub-optimal performance. Blame is allocated to either people or processes. The issue being experienced is actually the output or end result of a sequence of systemic underlying problems. We are witnessing the of tip of the iceberg and action corrective countermeasures are deployed at that which we see.

More often than not, departments implement newer/fresher rules to mitigate these threats. Rules such as independent reviews, tri-factor authentication, candidate screening, psychometric assessments, et.al. Rules are cascaded upon old paradigms and thus problems can and will eventually creep up.

And because of the number of problems and the complexity of solving these, you are often left to firefight each day. Each new day brings new complexities to the fold and solutions are merely temporary constructs to hold these at bay in order to tackle the next most immediate problem.
